Customise / edit PMH forms

Past medical history forms are unique forms that must be completed or reviewed in every patient journey

PMH forms are special types of questionnaire forms:

  • Every patient are asked to complete/review one during the patient journey.

  • If the patient has completed it before, the are just asked if anything has changed since their last visit rather than filling it in again.

    • If something has changed, the form will appear as it was previously completed ready to be updated rather than completed in full again.

  • Drug history, past medical history and allergies have been set up so that the answers to these questions automatically populate the PMH section in their digital notes. E.g. their allergy information will appear in the allergy section.

  • Drug history has a fixed extensive list of known common drugs

Collums consent forms cannot include questions - instead, ensure that your PMH form contains all the questions that you need to ask to cover all the treatments that you offer.

A PMH form is in your Collums account and contains many of the common questions but you may want to customise it or create a new one.

Create a new PMH form

The easiest way to do this is the copy the existing PMH form and update it as you wish. When doing this or adding a brand new form from scratch, check the "Is a medical history form" check box to ensure that answers sync with the Medical History, Drug History and Allergy History.

Edit the PMH form

Forms are accessed in admin/organisation/organisation settings/forms and require organisation level access.

  1. Click the blue PMH name in the list of forms and then click "Form Design" in the accordion.

  2. The form is built with different blocks.

    1. Blocks are listed on the right. Click on the + icon to the right of your desired block type to add it to the bottom of the form.

    2. Click on an existing block to edit it.

    3. Blocks can be re-ordered to appear further up or down the form (click the arrows to the left when hovering over a block).

    4. Blocks can be deleted (click the red trash icon to the right when hovering over a block).

  3. Click the blue "Preview" button to see how your form will look.

  4. When finished, click "Save"

Assigning the PMH form

In Admin > Services, you can now choose to change the PMH form from the standard, to any new form that you have created using the dropdown.
