The PMH form is a special type of questionnaire form:
Every patient is be asked to complete it during the patient journey.
If the patient has completed it before, the are just asked if anything has changed since their last visit rather than filling it in again.
If something has changed, the form will appear as it was previously completed ready to be updated rather than completed in full again.
Drug history, past medical history and allergies have been set up so that the answers to these questions automatically populate the PMH section in their digital notes. E.g. their allergy information will appear in the allergy section.

Collums consent forms cannot include questions - instead, ensure that your PMH form contains all the questions that you need to ask to cover all the treatments that you offer.
The PMH form in your Collums account contains many of the common questions but you may want to customise it.
Edit the PMH form
Forms are accessed in admin/organisation/organisation settings/forms and require organisation level access.
Click the blue PMH name in the list of forms and then click "Form Design" in the accordion.
The form is built with different blocks.
Blocks are listed on the right. Click on the + icon to the right of your desired block type to add it to the bottom of the form.
Click on an existing block to edit it.
Blocks can be re-ordered to appear further up or down the form (click the arrows to the left when hovering over a block).
Blocks can be deleted (click the red trash icon to the right when hovering over a block).
Click the blue "Preview" button to see how your form will look.
When finished, click "Save"
