Create form builder (questionnaire) forms

Form builder forms can contain questions in different formats in order to collect patient data

Form block types

  1. Title - adds centre justified title text.

  2. Paragraph - adds left justified title text (you can delete this if not required) and left justified paragraph text.

  3. Text input field - adds a text field that can be completed by the patient.

    1. Tick the "Required" checkbox to make completion of this field mandatory before being able to save the form.

    2. Use the drop down to have this text shown in the allergies, drug history or medical history section of the PMH section in the digital notes.

    3. Choose how you would like the text to appear in the section selected - as a label or in the description section. In the below, Asthma is the label, the text below is the description.

  4. Checkbox - adds checkbox with a text label that can be customised.

    1. Tick the "Required" checkbox to make completion of this field mandatory before being able to save the form.

    2. Tick the "Checkbox ticked as default" checkbox to have this checkbox ticked as default.

    3. Use the drop down to have this text shown in the allergies, drug history or medical history section of the PMH section in the digital notes.

    4. Choose how you would like the text to appear in the section selected.

  5. Radio buttons - adds customisable radio buttons for yes/no or multiple choice questions.

    1. Enter text for your question e.g. "Do you suffer with diabetes" or "How painful is it on a scale of 1-5?".

    2. Tick the "Required" checkbox to make completion of this field mandatory before being able to save the form.

    3. Options

      1. Each option is a radio button.

      2. Click the trash icon to delete a button.

      3. Click the pencil icon to edit the name of the button.

      4. Click an arrow to rearrange the order of the buttons.

      5. Compete the "Add another opinion" field and click the blue plus button to add a new button.

      6. Tick the "Default value" checkbox and select the buttons you wish to have selected as default.

      7. Show choices in Medical History Section?

        1. Use the drop down to have button text show in the allergies, drug history or medical history section of the PMH section in the digital notes.

        2. Choose how you would like the text to appear in the section selected.

  6. Dropdown list - similar to Radio buttons but with options in a dropdown list

    1. Enter text for your question.

    2. Tick the "Required" checkbox to make completion of this field mandatory before being able to save the form.

    3. Dropdown options

      1. Tick the "Choose multiple options" checkbox to allow selection of more than one option.

      2. Tick the "Use drug list" checkbox to have the BNF list of drugs appear in the dropdown.

    4. Show choices in Medical History Section?

      1. Use the drop down to have button text show in the allergies, drug history or medical history section of the PMH section in the digital notes.

      2. Choose how you would like the text to appear in the section selected.

  7. Radio with dropdown - Yes/No radio buttons (as above) but with dropdown when clicking "Yes"

    1. Enter text to appear to the right of the dropdown field if you wish.

    2. Autocomplete - when using BNF drug list - suggestions appear as field is completed so save having to complete the entire drug name.

  8. Radios with text field - as above but a text input field appears (rather than a dropdown) when answer is Yes.

  9. Dropdowns & text fields - as per the dropdown list but with a text field to input further information when a selection is made.

    1. Placeholder text - this is the text that appears in the text input field to prompt the information required e.g. "Please give more info". This text disappears when the field is selected for completion.

  10. Radio with Dropdowns & text fields - as above but with radio buttons - the dropdown appears on clicking the Yes button.

  11. Text Area - a larger text input field.

  12. Blank space - insert blank space to further separate blocks.

    1. Change the size to increase or decrease the height of the blank space.

  13. Image - upload and insert an image.

  • Play around with different blocks and settings - use the "Preview" button to see how it will look and behave when you select different options.

  • Use the PMH form as a guide. Copy it's settings when creating similar questions.
