Email consent (and other) forms to clients automatically on booking

Automate consent forms to be sent in advance of an appointment

Any form can be sent to a patient in advance of their appointment. These are sent at the time of booking.

  1. Click on Admin in the right hand menu.

  2. Click on "Services" in the Organisation Resources box.

  3. Click the service category (or service) and click on the Form section.

  4. Click the blue plus under "Forms to be emailed in advance".

  5. Select the form

    1. NB The form will have to have "Allow send by email" checked in admin/forms to show in the list.

  6. Choose whether the form is to be sent on the first appointment or every appointment (e.g. every time they attend for a laser hair removal course or just the first).

  7. Click "Save"

You must also ensure that the "Pre-appointment forms" notification has been set up correctly and is active.

Pre-appointment forms notification

  1. This notification is set up the same as other notifications.

  2. The merge tag {FormLinksList} will populate the link of all forms that have been connected to the service being booked.

Once the above has been completed, the notification is emailed to the client at the time of booking. When they click on the link(s) the chosen form(s) will open in the online booking app. When the form is signed and saved, it is immediately uploaded to Collums.

You can also email forms that do not require signing.
