Connect consent forms to services

You can connect consent forms to individual services or to entire categories.

  1. Go to admin/resources/service (or categories) and click on the desired service or category.

  2. Click to expand the "Forms" section.

  3. Select your form to appear in either the client or practitioner sides of the journey (or both). You can also select in which order it should appear by using the arrows. TIP: If you wish the patient to go through the consent form on their own (before seeing the practitioner), choose the client journey (no practitioner name will appear in this case). Selecting the patient journey will ensure that the form will appear after the journey is unlocked via the practitioner pin.

  4. If you wish your form to be emailed to your patient on booking, select the form from the "Forms to be emailed in advance" section.

  5. For the form to be emailed, you will need to ensure that the pre-appointment notification has been set up and is active.

  6. Click "Confirm".
