The different types of forms

Forms can be created to gather patient information or deliver information including consent forms for signing

Forms can be used in the patient journey, sent to patients at the time of a booking or viewed (+/- completed) via the client card.

There are 2 types of forms in Collums.

  1. Consent forms

  2. Form builder forms

Consent forms

Consent forms in Collums can only contain text and pictures. No questions can be asked - if you wish to do this, use the form builder.

If you wish to ask any medical questions, include these in the PMH (past medical history) form.

Form builder forms

These forms can include questions with tick boxes, dropdowns, radio buttons and more. They can be used to collect patient information, the best example of this is the PMH form.

PMH (past medical history) form

The PMH (past medical history) form is a questionnaire form built with the form builder but has special properties within Collums.

  • The PMH form cannot be deleted. We advise that you do not change its name.

  • It is shown in every patient journey

The first time a patient goes through a patient journey, they complete the PMH in full. In subsequent journeys, the patient is asked if anything has changed in their medical history.

If they selected "Yes", the will be presented with the PMH in the state it was last completed so that they do not have to complete the whole form again, just update the changes. If the patient selects "No", they will not need to complete the form again.
